At perfectcare, we like to focus on the long term results that we can offer to NDIS and TAC participants to ensure they get the care and support they need. This includes providing accommodation when needed.
We have several houses in Melbourne that are designed for a combination of Coordinated Hospital Discharge, Respite and Short Term stays and Long Term Accommodation, depending on the participants needs and plan provisions. Our properties are designated as Specialist Disability Accommodation, but we try to make them as homely as possible so that you get the best care possible and feel comfortable when you stay with us.
Coordinated Hospital Discharge
Exiting hospital and establishing the right supports and accommodation can be challenging.
At Perfect Care we take a personalized approach to those requiring complex care and supports. We establish care plans that suit your needs. Your care and supports are led and delivered by highly qualified team of managers/ team leaders/ supervisors and support workers.
- We provide 24/7 care. Our after-hours service is available 24 hours a day to respond to any urgent needs or emergencies.
- We coordinate with all stakeholders/health partners to ensure that your new home suits your needs.
- Manage the process of transitioning you back into the community to be as smooth as desired.
- Develop a person-centred support plan and roster of care that suits your needs and funding
- Work closely in collaboration with the partners and allied health professionals to deliver your supports and reduce the risk of re-admission
- Supports provided by highly trained support workers to deliver your 24/7 supports.
Respite and Short Stays
Sometimes you need to get out of your normal accommodation and stay somewhere else. This can happen due to building repairs, carer respite needs and many other reasons. This option is idea if you have accommodation needs for less than 14 days.
When it does happen, you will need to find a place to stay for a short time. Perfectcare ‘s Respite and Short Stay options could be the ideal solution for you.
- Work within your plan to provide the essential needs you have, even 24/7 support if required.

Medium Term Accommodation
Medium term accommodation is available for NDIS Participants who need a home for more than two weeks (14 days) but less than three months (90 days). This is normally provided for people that are transitioning into long term accommodation, but require immediate accommodation.
Medium Term Accommodation is appropriate for any of the following situations, but needs to be part of your NDIS plan.
- Transitioning to a more independent living environment
- No longer able to live in your current home
- Waiting for modifications to be made to your home
- Needing assistive technology or support equipment to be installed
- Awaiting for appropriate Specialist Disability Accommodation (SDA) to be available.
NOTE: this is an accommodation only service and does not include meals and support. If a Participant requires meals or assistance with daily activities, these must be included separately in their NDIS plan.
