Coordinated Hospital Discharge is the process of safely transitioning you from hospital care back into the community, with all necessary supports in place.
At Perfect Care, we ensure that when you leave hospital, the services, accommodation, and care you need are ready and available. Our goal is to support a smooth, safe transition while reducing the likelihood of hospital readmission.
This service is suitable for NDIS participants who are:
- Ready to leave hospital but require ongoing care and support
- Waiting for suitable long-term accommodation
- Transitioning from hospital into NDIS-supported living
- Requiring coordinated health, personal care, and support services
Safe Transition from Hospital
The first step in discharge planning is confirming your destination and arranging transport.
Perfect Care works closely with hospitals, families, and support teams to ensure you are safely transitioned from hospital into appropriate accommodation. Whether you are returning home or moving into supported accommodation, we coordinate transport and services to make the process as comfortable and stress-free as possible.
If long-term accommodation is not immediately available, we can organise suitable short-term options while a permanent solution is secured.
Coordinated Allied Health Services
Ongoing health support is essential after leaving hospital.
We coordinate a wide range of allied health services, including:
- Community nursing
- Medication management and prescriptions
- Therapy services and health assessments
These services help you maintain your health and independence without needing to return to hospital unnecessarily.
Our team works with experienced allied health professionals and ensures all supports align with your NDIS plan.
Accommodation Support
Having the right place to live is critical to a successful discharge.
If you are unable to return home, Perfect Care offers:
- Short-term accommodation (STA)
- Medium-term accommodation (MTA)
- Specialist Disability Accommodation (SDA)
We ensure you have a safe and supportive place to stay while you explore long-term housing options. Our team can also assist you in visiting and selecting accommodation that suits your needs and preferences.

Perfect Care provides personalised care services, including:
- Assistance with daily routines

Our Supported Independent Living (SIL) services are available both in our accommodation and in your own home, ensuring continuity of care as you transition.
Ongoing Support Coordination
Your needs may change after a hospital stay.
Our experienced NDIS Support Coordinators work with you to:
- Review your current supports
- Identify any gaps in your care
- Utilise existing plan budget to access supports that are better suited to the participants changed needs. If there is significant change in circumstances following hospital admission – support coordinator can assist participant to apply for “ Change in Circumstance” Or apply for Plan review if that is appropriate due to changes and increased supports now required.
This ensures you continue to receive the right level of support as your situation evolves.
