Supported Living

At perfectcare, we like to focus on the long term results that we can offer to NDIS, TAC and Worksafe participants to ensure they get the care and support they need. This includes In-Home care. The NDIS term this Supported Independent Living, while TAC call it a Daily Routine.

We provide a range of in-home care needs to individuals, ranging from nursing care to daily routine support and complex care asssistance. In addition, we specialise in coordinated hospital discharge, Positive Behaviour Support and Youth Services.

In-Home Care (SIL)

Many people need assistance with their personal hygene and/or medicines on a daily basis. This is often refered to as a daily routine, but in reality it is a regular scheduled agreement to provide assistance based on personal needs. Normally, the level and frequency of assistance is defined by the organisation coordinating the support (NDIS, TAC or Worksafe) and is tailored for everyones unique circumstances. This type of support is provided at your home.

Specialist Care

Although PerfectCare provide a lot of services that other provides also provide, we have identified a few services that require specilaist attention. For us, we specialise in:

  • Coordinated Hospital Discharge – the coordination and support of participants being discharged from a hospital. This includes working with medical staff and organising accommodation and care as required. The main objective is to prevent readmission  by ensuring proper support is provided before discharge occurs.
  • Behavioural Support – Specialist support for participants with behavioural issues that can result an unexpected and sometimes violent reactions. The main objective is to prevent harm to participants and others when behavioural issues occur.
  • Youth Services – People that are still growing and developing have specialist needs to assist them in reaching their potential without loosing themselves in mundain routines. This is especially true when something is different about them and they need to know they are valued and important.
