At Perfect Care, we are a Registered NDIS Provider offering a range of disability services to NDIS Participants in Dandenong, a South Eastern suburb of Melbourne. Our aim is to make a difference in the lives of Participants with disability in Dandenong. We do this by providing individual and personalised support 24/7, 365 days a year. Our goal is to make each of our Participant’s life easier and more equitable, encouraging greater involvement in their communities.

The range of services we provide include Supported Independent Living accommodation close to Dandenong. This includes the suburbs of Clyde North, Cranbourne North, Cranbourne West, Lynbrook, Narre Warren South and Noble Park. We also provide short term (respite) and medium term accommodation in our homes.

Some of the other services we provide include assistance with household tasks, daily personal activities, transport, community activities, and nursing care. We also assist with support coordination.

NDIS Accommodation

Perfect Care Supported Independent Living (SIL) accommodation offers a range of options for NDIS Participants. This includes both children and adults living with trauma, physical disabilities, developmental delays, and autism spectrum disorder (ASD). We also specialise in providing NDIS SIL accommodation options for Participants with psychosocial and dual diagnoses.

In addition, we offer short term (respite) and medium term accommodation in our homes. Our fees cover all expenses for a 24 hour period.

NDIS Independent Living Support Dandenong

Whether it’s grocery shopping, cooking meals, or helping with laundry tasks such as washing, drying, and ironing, we’re ready to assist. We also help with bed linen changing, dishwashing, wiping down benchtops, and maintaining cleanliness around the home. In addition, we offer mopping, dusting, and vacuuming to ensure the living space of your Dandenong home is comfortable and clean. NDIS Daily Personal Activities Support Dandenong

At Perfect Care, we assist and/or supervise your daily personal activities to ensure all your care needs are met, including high intensity care provided by a TTP Registered Provider. This support is available for Participants in supported accommodation or those living in their own homes in Dandenong.

Our NDIS services include personal hygiene, toileting, bladder and bowel management, eating and drinking, mobility and transferring, the use of aids and appliances, and complex care needs such as enteral feeding, tracheostomy management, and wound care. Our goal is to help you live as independently as possible while meeting your immediate care needs, with assistance available day and night, including sleepovers and active overnight stays for high intensity care.

Additionally, we offer community nursing services like wound care and medication management to support your health and well-being. Our team collaborates with healthcare professionals to ensure you receive the best care in your home in Dandenong.

NDID Support Coordination Dandenong

At Perfect Care, we provide NDIS support coordination to help you review and improve your NDIS Plan, both during the application process and as ongoing support. Our focus is on ensuring that Participants in Dandenong receive the guidance and resources needed to maximise their NDIS funding.

Our services include identifying the right supports, negotiating services, pricing, and agreements, and managing schedules, budgets, and reports. We also liaise with NDIS Plan Managers to keep your plan on track.

If issues arise with plan implementation or service delivery, we work to resolve them promptly. Additionally, we review your plan regularly to assess success, value for money, and future improvements. Our aim is to help you achieve your goals and adjust your plan as needed.

Travel and Transport Dandenong

At Perfect Care, we have a range of vehicles to transport Dandenong NDIS and TAC Participants to their destinations. Our fleet consists of five vehicles, one of which is specially equipped with a wheelchair loader to provide accessibility and comfort for those with mobility needs.

We provide transport to a wide range of destinations, including within Dandenong as well as other parts of Melbourne. Some of the places we transport Participants to include medical appointments, work and education, community and social activities, shopping, recreational activities, therapy or rehabilitation, and family visits or social outings.

Additionally, we assist NDIS Participants with using public transport, ensuring they have access to the resources they need for independent mobility.
